Certificate of Child Health Exam
Attention Kindergarten, 6th grade and new students to Enders: if you have not yet submitted your child’s Certificate of Child Health Exam, please do so as soon as possible. A blank copy to be completed by your provider can be found here on the SD54 website. School physicals and immunizations must be up to date by October 15 according to the Illinois School Code. If you have been notified your child is missing a physical or immunization and you do not have the required documentation or written proof of an appointment, you will be called to pick your child up from school on October 16.
