School District 54 requires that any student who participates in a team or intramural sport must have a current, approved physical on file in the health office prior to tryouts. The physical must be administered by a physician or nurse practitioner and is valid for one year from the date of exam. You can find the Physical form here: English and Spanish.
Please make a copy of the physical for your own records before submitting it to the school.
** The Health History (located on the back of form) MUST be completed and signed by the parent for the physical to be complete.**
